Mikael is the doctor who is on call overnight. This is a career path. Between his two patient visits in poor neighborhoods, he provides medical attention to people that no one else wants to treat, such as drug users and homeless people... He associates with people who are destitute. His life is a complete mess right now. Especially when it comes to his cousin who is a pharmacist and forces him to write fake prescriptions for the drug Subutex. Overnight, he makes the decision to walk away from the drug trade and begin a new chapter in his life. But there will be a significant cost associated with this decision.
