Originally, Dr. Caine was a dentist who killed people. He has now escaped from the mental hospital where he's been since he was caught. It isn't easy for him to get his life back on track, but he makes his way to a small Midwestern town and pretends to be the town dentist. When Caine sees his new love in someone else's arms, things start to look up for him. When this happens, it sends him back over the edge and into another killing spree, with his victims taking the brunt of his anger.
