I Give My First Love to You
As a child, Takuma is told he has a heart problem that needs to be taken care of by a cardiologist. During his time in the hospital, he quickly makes friends with Mayu, the young daughter of his doctor. Mayu overhears her father telling someone else that Takuma's illness is so bad that he probably won't live past the age of 20. Takuma doesn't know this, though, so when he's 8 years old, he promises to marry Mayu when they're both 20. Later on, Takuma realizes that he won't be able to keep this promise, so he starts to stay away from Mayu to protect her.
